INITIAL VISIT POLICY: All first time appointments require a $100 deposit (which is used towards the initial visit) before booking. This deposit is non-refundable if you No-Show or Cancel/Re-Schedule your appointment less than 24 hours before your appointment.

LATE/CANCELLATION/NO-SHOW POLICY: Please try your best to be on time so that we adhere to the time allotted to you and respect our time as well as the time of all practice members. We understand life happens, and will do our best to extend grace to everyone we serve. If you arrive late, your appointment time will be shortened to accommodate every practice member. If you arrive after your scheduled appointment, you will be rescheduled and CHARGED HALF the visit fee. If you cancel or re-schedule less than 24 hours before your appointment you will be CHARGED HALF the visit fee. If you completely no-show (including not contacting at all before the end of your appointment) without notice you will be CHARGED for the visit IN FULL.

INSURANCE & THIRD PARTY POLICY: Our office is not in network with any health insurance providers. However, we do accept Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) as forms of payment in our office. We can provide you with an itemized receipt for you to submit independently to your insurance company. We do not provide any billing or submission of paperwork on your behalf to your insurance company. You are responsible for payment in full at the time of your visit. Additionally, our office does not take auto accidents and do not work as a third party provider with insurance companies or law firms.
